  • Health Homecare assists people with substantial physical disabilities by maintaining or increasing their independence in their homes by providing support with personal care and activities of daily living. Meal preparation, light housekeeping, bathing, dressing, and grooming, medication set-up and supervision, respite care, advanced personal care, ostomy care, catheter care, bowel program, and transportation are just some of the services available.

  • In order to receive DMTLC health services, clients must be eligible for Medicaid with some requirements: Clients must be enrolled in the Program for the All-inclusive Care of the Elderly (PACE) must access home health services through PACE. Clients who have elected hospice care must access home health services through the hospice benefit. However, children under age 21 who are receiving concurrent curative treatment with hospice care may receive home health services related to the curative treatment. Clients enrolled in a Medicaid managed care plan must access home health services through their managed care plan.
